Every update for the year — new features and fixes as they ship, the same builds this server runs. Plus license activation and the download servers your instance pulls from. Self-hosted doesn't mean self-sufficient; my side keeps that running, and that infrastructure isn't free for me either. It's the price of keeping it working, not a license fee for the software.
Straight answer: your server keeps running. It stays on the version it has, with all your data, for as long as you want to run it. Nothing shuts off and nothing phones home any more.
What you lose is updates — no new features or fixes until you renew. You get warnings for the last two weeks before expiry. Renew whenever you like and updates resume; there's nothing to reinstall.
